Complete and submit the application electronically, using the How To Apply page. Do not exceed the limit on length and do not expand answers onto additional attachments. The only page that may be expanded is the budget form.
We require that BOTH the Board Chair and the Executive Director enter their names and emails on the final page of the application to certify that they have reviewed and approve of the application. If the organization’s management-structure differs, please supply appropriate information from those individuals with corresponding positions, or contact Foundation staff for guidance. Also, please note that the same person may not enter their name in both boxes (even if this person is both Executive Director and Board Chair) – in this case, the signature of a different board officer may be substituted for the Board Chair. For Poudre School District requests, we require the approval of the grant originator and either the Superintendent, Department Head or school Principal as appropriate. ***Please be advised that both signing parties will receive an e-mail notification certifying receipt of application.
