Application Instructions

  • Complete and submit one hard copy of the application. Do not exceed the limit on length and do not expand answers onto additional sheets of paper. The only page that may be expanded is the budget form.
  • Only one copy of the application is necessary. The copy does not need to be in color. Do not put applications in binders or folders as they will be discarded. Do NOT include additional materials; the additional materials will not be reviewed.
  • Sign the application, noting we require the signature of BOTH the Board Chair and the Executive Director.

    • If the organization’s management-structure differs, please supply appropriate signatures from those individuals with corresponding positions, or contact Foundation staff for guidance

    • Also, please note that the same person may not provide both signatures (even if this person is both Executive Director and Board Chair) – in this case, the signature of a different board officer may be substituted for the Board Chair.

    • For Poudre School District requests, we require the signature of the grant originator and either the Superintendent, Department Head or school Principal as appropriate.

The following attachments are required:

  • IRS 501(c)(3) determination letter (government entities excluded)
  • A completed and signed application
  • A detailed budget for the entire project or program you are proposing (not required for General Operating requests)
  • The organization’s budget for the year in which the grant request is being made
  • Externally prepared financial statement (audited, reviewed, compiled). If unavailable, provide the most recently filed 990 report
  • A complete list of the organization's Board of Directors
  • A self-evaluation will be required of all grantee organizations within sixty days of project completion or after 14 months from the date of the grant for ongoing projects or prior to any new application for funding for the project for which funding has been previously awarded. The Bohemian Foundation may also initiate an additional evaluation or assessment as deemed necessary.
  • The application must be RECEIVED by 5pm on the deadline date. Applications may be mailed or hand delivered. Applications postmarked prior to the due date but received after will not be accepted. Applications received via e-mail or fax will not be accepted.

It is important for applicants to remember that there are a range of IRS guidelines on the receipt of funds by any non-profit. Each applicant MUST accept responsibility for understanding the potential impact on their non-profit status of receiving funds from any source. You should contact your tax adviser if you have any questions regarding the public support requirements.

As an example, for any organization that has been classified by the IRS as a public charity because of the public nature of its support (i.e., under Section 509(a)(1)/170(b)(1)(A)(iv) or Section 509(a)(2) of the Internal Revenue code), total grant awards - within the calendar year - may not exceed 30% of the organization’s annual budget or otherwise hinder its ability to meet its public support requirements.

Publicity Standards

While we encourage you to publicize your organization or project the Bohemian Foundation assisted in funding, please note in the letter agreement that you are required to obtain approval from the Bohemian Foundation prior to the printing or distribution of any publicity materials that reference the Foundation, including but not limited to: press releases, advertisements, signage, and event collateral. For all logo requests please include the size and file format needed.

Please allow 10 days for Foundation staff to review and approve any proposed publicity.

Please send all publicity and logo approval requests to press@bohemianfoundation.org. Please note, no changes to the Foundation logo are allowed without prior written approval.
