| Evaluation is important to the Bohemian Foundation because it helps us learn from our grants, and in turn, become better grant-makers. Participation is critical to the success of this process. Please take the time to fill out this form and return it to our office. | |||||||||
| • Please complete the Colorado Common Grant Report Format (CCGRF) cover page in full. • The Final Report due date is sixty days following the end of the grant term as stipulated in paragraph one of the grant letter agreement. • If you have an outstanding final report from a previous grant award for a program for which you plan to apply, a final report using the Colorado Common Grant Report Format must be completed and submitted along with,or prior to, any new application for funding. • Please repeat all questions and answer, even if the answer is “N/A”. • If the grant award is less than $1,000 please complete the Common Grant Report Format under $1,000 and answer only those questions marked with an “X”. • Except in cases where you are required to submit a final report with your application (see above), if the grant award received is for a specific program, please do not complete the evaluation until you have final actual numbers to compare to ;the budget submitted with your Pharos Fund application. If this is not possible within the two month timeframe that the evaluations are due, please call us to discuss the grant and timeframe. | |||||||||
