| Bohemian budget form (download Excel spreadsheet version) The Bohemian budget form is required for all Pharos Fund grant submissions. The Bohemian budget form provides information on both expected funding sources and its uses for the entire program/project cost for which the request is being written. | Certificate as to Tax-exempt Status (download PDF version) As a result of Federal tax legislation adopted by Congress in the summer of 2006, we now require all grant submissions to be accompanied by the Certificate as to Tax-exempt Status (government entities excluded). There are instructions attached to the Certificate as to Tax-Exempt status itself. Please read these instructions and consult with your legal advisor if you should have clarification questions. |
